it has never been revealed in the anime who left the paper note in takumi's shoes or called him to go to the restaurant that nite, before the first battle with EVO3. i cannot recall the episode where it was shown, bcoz i do not have any copies of 2nd stage. it might have been told in the manga, but i dont know bcoz i never read any of the comics before. ask those who are familiar with the comic story. there maybe an answer there. as far as i remember, the caller was never revealed in the anime.

we're hoping it wont be a total of 16 episodes. 17 & 18 for sure will come out, bcoz they need to wrap up takumi's battle. after that, we would just have to cross our fingers. since the habit of japanese anime producers are either : 13 episodes for half a season / 26 episodes for a full season / 100++++++++ episodes for good, supported shows, (less than 13 due to cancelling/special OVA), i think its safe to expect 26 4th stage episodes (or at least 22?) since they're past the 13 mark. unless the show is cancelled of course :p
if i remember correctly, hellsing was also 13 ep. or was it 11? :D
